The San Antonio Food Bank will be offering application assistance onsite to those interested in applying for:

• SNAP (formerly known as food stamp program)
• CHIP
• TANF
• Adult Medicaid/WHP
• MEDP
• WIC

Bring a copy of the following information to apply:

• DL or ID and Social Security numbers for everyone in the home
• Proof of all income: Last 3 Paystubs, Social Security award letter, etc
• Proof of address if different from DL/ID
• Proof of expenses (i.e. rent/mortgage, water, light, etc)
• Birth Certificate and Immunization Records (TANF and Children’s Medicaid only)
